Fax the encounter notes, demographics, and orders to (408) 998-8005, or call our intake line directly at (408) 998-0550.

We coordinate care plans with the physician
The physician receives the plan of care, treatment, and goal orders for approval. We update the physician when patient symptoms are outside parameters, via written orders and phone calls.
