Skilled Nursing
Medication management, disease management, education processes, wound care teaching, ostomy care/instructions, and pain management — performed by skilled nurses in your home.
Physical Therapy
Our licensed physical therapists provide comprehensive in-home care designed to restore strength, mobility, and independence. We hold specialized certifications, including LSVT BIG (Lee Silverman Voice Treatment) for patients with Parkinson’s disease and NDT (Neuro-Developmental Treatment) for stroke recovery, ensuring patients receive evidence-based, condition-specific care tailored to their unique diagnosis.
Occupational Therapy
Our licensed occupational therapists help patients regain the skills needed for daily living and greater independence, with individualized treatment plans that may include training in activities of daily living (ADLs), fine motor and coordination exercises, cognitive rehabilitation, and home-safety assessments. We offer specialized care, including LSVT BIG, NDT, and hand therapy, ensuring evidence-based treatment tailored to each patient’s diagnosis.
Speech Therapy
Our licensed speech-language pathologists help patients regain communication, cognitive, and swallowing abilities. Each therapist conducts a thorough evaluation and develops an individualized treatment plan that may include speech and language exercises, cognitive-communication training, and swallowing (dysphagia) therapy to reduce the risk of aspiration and to improve safe eating and drinking.
Medical Social Work
A licensed social worker helps families navigate financial resources, in-home support services, community resources, and long-term care planning when needed.
Home Health Aides
Trained aides assist with bathing, dressing, grooming, and personal hygiene — under the supervision of a licensed clinician and the physician’s plan of care.
